Heartland Alliance Health is a mission-driven Federally Qualified Health Center (FQHC)
Committed to improving health equity for underserved populations across Chicago. With a person-centered, whole-health approach, we deliver high-quality, accessible care that supports the physical, mental, and social well-being of our participants.
Who We Serve
We serve individuals and families, including those who are low-income or unhoused.
Support is available to help participants navigate insurance and benefit options. No one is ever denied care. Through partnerships, compassionate care, and tailored support, we are building stronger, healthier communities—one person at a time.
Services
Our Comprehensive Services
Case management, outreach, and support services.
Primary Medical Care
Routine Check-ups preventive screenings, chronic disease management, and pediatric services.
Behavioral Health
Individual and group counseling, psychiatric services, and treatment for mental health and substance use disorders.
HIV/AIDS & Infectious Care
Comprehensive support for individuals living with HIV/AIDS, including testing, prevention, and ongoing care.
Nutrition & Wellness
Food and nutritional counseling, chronic disease education, and wellness support for long-term health.
Specialty Referrals
Access to a network of trusted specialists for coordinated care beyond our clinics.
Insurance
We will see you—regardless of whether you have health insurance.
If you have Medicaid or Medicare, we accept most plans.
If you do not have Insurance:
You can apply for Medicaid or renew your Medicaid coverage through the IL Dept of Healthcare and Family Services at: hfs.illinois.gov. - We can help you Enroll or re-enroll in Medicaid choose a Medicaid program (MCO). Determine your eligibility—and help you sign up—for benefits and entitlements you may be eligible for troubleshoot issues with your health benefits
- We operate on a sliding fee scale as a Federally Qualified Health Center, our fees are based on the relationship between your income and your ability to pay for care. Our fees are calculated using the annual poverty guidelines published by the U.S. Department of Health and Human Services. But because our clients typically fall far below the poverty line, they do not meet the minimum income standards for a co-pay for our services
